Life Care Center of Raynham, Massachusetts, achieved a deficiency-free federal survey in January 2018.
To pass the survey, nursing facilities must meet hundreds of criteria. The survey focuses on a wide variety of care areas in the facility, including nursing care, patients’ rights, safety, food services, administration and the physical condition of the building. Surveyors spend several days in the building reviewing paperwork, observing resident care and interviewing residents and associates.
“Our staff did a great job,” said Phyllis Dearing, director of nursing. “Everyone here is invested with the commitment to being the premier provider of nursing and rehab in the community, and they work very hard.
